Long term care” means helping people of any age with their medical needs or daily activities over a long period of time.

Long term care includes an array of health, personal, social, and supportive services needed by individuals who have lost some capacity for self-care. 

The HealthCare Center at West Meade Place offers care to people who cannot be cared for at home or in the community.
We provide:

  • Nursing care
  • Meals
  • Activities
  • Help with daily living
  • Supervision

We can also assist with:

  • Eating
  • Bathing
  • Dressing
  • Grooming
  • Going to the bathroom
  • Walking
  • Transferring
  • Remembering to take medicines

Health care needs can also include medical nutritional therapy, oxygen, health monitoring (for diabetes, for example) and pain management. This type of care can be in lieu of hospital care, after hospital care, or to give family or friend caregivers some time off (“respite care”).
